Grand Old Lady Scenario Questions

We tried to play this scenario (part of the Campaign Pack from D-Day) yesterday and had some questions:

1. The scenario lists Mustang IA’s … where does one find a card for the Mustang IA?

2. The scenario lists two pilot skill levels for the three plane element, what is the skill level of the third pilot?

3. Where does one find the Head-On Attack Doctrine card for the Luftwaffe?
